The EDS Clinic is a virtual-first clinic specializing in rare and complex conditions such as Ehlers-Danlos Syndrome (EDS), Postural Orthostatic Tachycardia Syndrome (POTS), Mast Cell Activation Syndrome (MCAS), and related neuroimmune and connective tissue disorders. We provide compassionate, evidence-based care to patients across the U.S., reducing barriers to specialized care.
We are seeking a highly organized, empathetic Care Coordinator to support our growing patient population. This role is ideal for someone who thrives in a fast-paced remote environment and is passionate about improving healthcare access for patients with complex needs.
Key ResponsibilitiesPatient Support & Communication
- Serve as a central point of contact for patients, responding to messages via EHR, phone, and secure chat.
- Provide timely support related to scheduling, referrals, care navigation, and prescription coordination.
- Monitor patients’ progress across the care journey and follow up on outstanding tasks or concerns.
- Escalate clinical or urgent issues to providers appropriately and promptly.
Care Coordination & Navigation
- Coordinate between patients, internal clinical teams, and external providers to ensure seamless transitions of care.
- Help patients navigate diagnostics, lab testing, imaging, and specialist referrals.
- Track and document the status of tasks such as prior authorizations, follow-up appointments, and patient-reported outcomes.
- Maintain updated records of care plans, follow-ups, and patient communications.
Operational Excellence
- Maintain detailed, HIPAA-compliant documentation in the Electronic Health Record (Elation).
- Participate in regular team meetings, process improvement efforts, and workflow audits.
- Identify and flag gaps in care or potential patient barriers (logistical, financial, systemic).
- Support intake processes and assist with onboarding new patients to ensure a smooth experience.
Qualifications
- Minimum of 2 years in a care coordination, case management, or healthcare operations role.
- Experience with chronic illness populations, especially in neurology, immunology, cardiology, or rare diseases, is preferred.
- Strong communication, multitasking, and relationship-building skills.
- Proficiency with digital tools, including EHR systems, Google Workspace, and patient messaging platforms.
- High attention to detail and ability to manage multiple active cases with sensitivity and care.
- Comfortable working independently in a fast-paced, virtual care setting.
Contract Terms
- Employment Type: 1099 Independent Contractor
- Hourly Rate: $15–$30/hour (depending on experience and qualifications)
- Schedule: Flexible, with preference for consistent coverage within traditional business hours. Morning coverage is especially valued.
- Location: Fully remote; must be based in the U.S. with reliable internet and a private workspace.
